Moondragon and Morbius's origins

Some of the page numbers are wrong in the FB telling Moondragon's origins. It seems that for some issues, the ads are counted in the page numbers. For example, there's a DD 105 (23:5 - 23:8)-FB though DD 105 is only 19 pages.
Thanos's listing has the correct page numbers for the DD 105-FB. Mentor and Starfox's listings don't have page numbers.

Checking those page numbers, I noticed Moondragon's first chronological appearance is missing.
A 135 (9:2)-FB is Heather walking out of the car after the accident
CM 32 (11:10)-FB is Heather in the car before the accident
